CEO and top executives or wealthy people who can afford it. It’s still worth it for red eye flights and long international flights (6+ hours).
Having said that, a lot of people can actually afford to fly in first class. As an example, it would cost 140,000 miles to fly first class from Hong Kong to New York (round trip). It’s definitely achievable to get this amount of points with your day to day spending and opening new credit cards.
First class on Air France, Lufthansa, Singapore Airlines or Emirates is a lifetime experience that you should try. It’s true luxury and you will enjoy every minute of it.
For those who believe that american airlines provide first class service, you should consider flying international on european or asian airlines. Delta First Class service in the US (domestic flight) is subpar compared to Lufthansa or Air France for example.
